To receive the benefits of a CAPS membership, choose one of the methods of enrollment below:
- 1. Instant Enrollment
- Instant enrollment over the telephone with a major credit card is available by calling 952-857-1177. Please have your Visa, Mastercard, American Express or Discover Card available. Phone lines are open Monday-Thursday 9-5 and Friday 9-4.
- 2. Mail Form
- Complete, print and mail the form below to CAPS along with the appropriate membership dues. Make checks payable to CAPS, then print and mail them to:
